I have been printing off the AMCAS transcript request forms. Many schools have their own request forms. Is it advisable to fill out a copy of the school’s request form along with the AMCAS one?
Also, I did my coursework this past year at a branch campus. When AMCAS prints out the transcript request, it lists the registrar’s office at the branch. However, I think all transcripts go through the registrar at the main campus. Should I change the address on the request?

The AMCAS instructions tell you to send their request along with the transcript. What I did was to fill out the AMCAS request along with the school request and note that the AMCAS paper should be sent along with the transcript. Good luck.
